The Wire, Boardwalk Empire and Lovecraft County star Michael K. Williams' official cause of death has been revealed.

As previously reported, the actor passed away in Brooklyn earlier this month on September 6. He was 54.

According to TMZ, New York City's Chief Medical Examiner has ruled Williams' death as an accidental overdose. The actor reportedly ingested a lethal mixture of fentanyl, cocaine, heroin and p-flurofentanyl.

Williams' overdose has also spurred a criminal investigation by the NYPD to locate the source of the drugs that caused his death.

Earning five Emmy nominations over the course of his career, the actor was also known for his roles in 12 Years a Slave, The Road, Gone Baby Gone and Inherent Vice.
